Understanding the New 2025 DAS Intubation Guidelines: What They Mean for Airway Training 

The Difficult Airway Society (DAS) has published its 2025 Guidelines for the Management of Unanticipated Difficult Tracheal Intubation in Adults, marking one of the most significant updates to airway management practice in the last decade. The updated algorithm simplifies the escalation pathway: 

A – Tracheal intubation (maximum 3 + 1 attempts) using a video laryngoscope 
B – Second-generation supraglottic airway (up to 3 attempts) 
C – Two-person mask ventilation with adjuncts and optimal positioning 
D – Declare CICO → front-of-neck access using the scalpel–bougie–tube technique .

– Situational awareness 
– Clear role assignment 
– Declaring failure early
